Airborne Hazards and Open Burn Pit Registry

Registry is open to any veteran or service member who served in OEF/OIF/OND or in Djibouti, Africa, after September 11, 2001, or Operations Desert Shield or Desert Storm or the Southwest Asia theater or operations after August 2, 1990.

How do you participate?

C-123 Aircraft Agent Orange Exposure

Any Air Force Veteran or Air Force Reservist personnel that were assigned to one of the affected aircrafts and developed a disability linked to agent orange exposure should file a claim.

How to apply:

Apply online through the VA Benefits portal. Fill out VA Form 21-526

  • Be sure to list each C-123 related disability

  • Include, if available:

    • Discharge paperwork (DD214)

    • USAF Form 2096 (assignment)

    • USAF Form 5 (aircraft flight duties)

    • USAF Form 781 (aircraft maintenance duties)

    • Dependency records (marriage and children's birth certificates)

    • Medical evidence
